Introduction

Questionnaire is a document that is filled out during a survey event.
Each sheet consists of topics, and topics consist of questions.
In list of operations at point of sale, questionnaires are marked with icon .
Before filling out the questionnaire, you need to begin a visit to outlet.

Calling dialog form:  > Route > Outlet > Questionnaire

Appearance of questionnaire

Questionnaire can be presented in page or tabular form.

Page form

In page form, topic views are displayed as pages.
Questions of selected topic follow each other in form of list.
Name of topic is displayed at the top of timeline, movement between topics is carried out by buttons Forward > and < Back.

To quickly jump to particular topic, click on page title, so list of survey topics will open.

Tabular form

In tabular form, topic views are displayed in table header (columns), and questions and answers are displayed in rows.

Mandatory questions

Mandatory questions are highlighted with a red frame.
If at least one of these questions is not answered, then it is impossible to save questionnaire.

Filling out questionnaire

1. Select cell (at intersection of question and topic).
2. Depending on type of question, you will see:
   • calculator,
   • dialog with choice of values,
   • text form,
   • calendar,
   • screen for choosing photo.
3. Enter the desired value.
4. Click OK.
5. After answering all necessary questions, click Next.

Photos in questionnaire

In ST-Mobi.com in properties of question, you can set answer type into photo.
Or photo should be added to main answer (as confirmation of answer, to check its correctness).

Adding Photos

1. Select cell with answer to the question, screen for working with photos will open.
2. Add one or more photos.
3. To take new photo, click on camera icon.
4. To add photo from the ones available on device, click on ellipsis.
5. In context menu, select Add from gallery.
6. Folder for storing photos will open.
7. Set flags next to desired photos.
8. Click Save.
9. After adding all necessary photos, click Save.

Deleting photos

1. Select cell with answer to the question, screen for working with photos will appear.
2. Set flags next to those photos that you want to delete.
3. Click on ellipsis.
4. In context menu, select Delete.
5. In dialog box that opens, click Delete.

Standard answers to questions

If question (regardless of its mandatory) cannot be answered or question was skipped, then one of the standard options can be chosen as an answer (no answer, refusal to answer, question was not asked).

1. Tap and hold the area of answer to question, so list of standard answers will open.
2. Choose one of the answer options.

Field in which standard answer is specified is highlighted with green border.

Filling in responses with historical values

As an answer to question, you can use answer specified when you last filled out questionnaire (historical value).
To do this (when creating a questionnaire template in ST-Mobi.com ) in properties of question, you need to specify type of history:

• in context of route,
• in context of outlet.

In this case, you can fill in answers with historical values.

Automatically:  > Settings > SKUs and Surveys > Automatically fill in answers to questions.
Manually: Questionnaire >  > Autofill.

Restrictions on data entry

When filling out the questionnaire, system checks answers for compliance with acceptable values.
If an incorrect value is specified as response (for example, incorrect date and time), system does not allow you to save response.

Setting column width

In questionnaire with tabular view, you can adjust width of column with questions (for example, if column is too narrow).

1. Tap and hold name of survey topic to open input form.
2. Specify new column width value.
3. Click OK.

In questionnaire with a page view, column width is adjusted automatically, depending on width of topic title.

Additional features

Calling dialog form: by clicking on button  on document screen.

Sending collected data to e-mail

After information on questionnaire is collected, employee will have opportunity to send completed document to specified email address in mobile application.

Final report

After completing document, click Next, final report will open.

Saving document

To save document, in final report, click Save.
System would send questionnaire to ST-Mobi.com with next data exchange session.
If answer to mandatory question is not filled in, system will display an error message and will not save the document.

1. Questionnaire is not visible on mobile device, what should I do?
   It is necessary to check several points in ST-Mobi.com:

   • period of activity of questionnaire template,

   • retail outlets and employees to whom questionnaire is distributed,

   • agents workflow (perhaps scenario was created earlier in which there is no new questionnaire).

2. How can I change form of questionnaire from tabular to page?
   Appearance and form of questionnaire is set in settings of questionnaire template in ST-Mobi.com.
3. How can I disable ability to add photos to questionnaire responses from memory card of mobile device?
   To disable this feature, there is a setting in ST-Mobi.com named Ability to add photos from file system and you need to set it to 0.
